Abstract Background Choledochal cysts are uncommon congenital biliary duct dilatation involving either intra-hepatic, extra-hepatic ducts or both. The presentation varies among pediatric age groups compared to adults, in which pediatrics present mostly with jaundice whereas adults abdominal pain. It carries high-risk complications and related malignancy depending on the type of cyst. There were few reported cases spontaneous rupture choledochal cyst adults. Case Description A 35-year-old female, medically surgically free. Presented Emergency Department recurrent episodes non-bilious vomiting for three weeks, managed conservatively. One day later, she presented generalized pain persistent vomiting. patient mentioned a history chronic 6 years. On examination, abdomen was tender all over signs peritonitis. Computed Tomography scan revealed septated intra-hepatic measuring 9 x 11 cm. Therefore, taken operating room exploratory laparoscopy, washout done drains inserted. Afterward, Endoscopic Retrograde Cholangio-Pancreatography sphincterotomy stenting performed. Six weeks underwent elective left hepatectomy histopathology confirmed She had an uneventful postoperative course two follow up no complications. Conclusion Ruptured is rare high clinical suspicion warranted adult patients who surgical along This case report represents importance conservative treatment before definitive operation, allow resolution peritonitis lower consequences.
